I understand that yoga is a gentle exercise system, requiring movements and minor flexibility. Participants do so at their own risk. Yoga at Everyday Wellness accepts no liability resulting from this exercise program. I understand that yoga classes are available to clients and non-clients of Everyday Wellness. Therefore, everyone does not have the same ethical and legal rules of confidentiality. Please be mindful of what you share in this setting as confidentiality may not be kept. I understand that Everyday Wellness providers may attend yoga classes. However, mental health clinicians will uphold the confidential agreement signed upon in the intake and all details of private sessions will remain protected. I understand that yoga is free of charge for active Everyday Wellness clients. Active is defined by obtaining services from our Licensed Professional Counselors or Massage Therapist within a 90 day period of the scheduled class. I understand that the Yoga Instructor does not diagnose illness, disease or any other mental or physical disorder. The Instructor does not prescribe medical treatment of pharmaceuticals, nor does she perform any spinal manipulations. It has been made clear that the yoga exercises that are undertaken are not substitutes for medical examination or diagnosis and that it is recommended that I see a medical practitioner for any physical ailment that I may have. I understand that services offered today and in the future are not a substitute for medical care and that any information provided by Everyday Wellness Yoga Instructor is for educational purposes only and is not diagnostically prescriptive in nature. I realize that it is solely my responsibility to keep Everyday Wellness Yoga Instructor updated on any changes of my physical health and I understand that Everyday Wellness and the practitioner shall not be liable should I fail to do so. I understand that all yoga services provided are strictly non-sexual. By signing this release, I hereby waive and release Everyday Wellness, LLC and our Yoga Instructor from any and all liability present, past and in the future relating to yoga exercises.
